Referrals
Dudley Children's SLT service has an open referral system. This means that anyone (including parents) can refer a child to the Service by contacting the Speech and Language Therapy department, if a child has an identified Speech, Language, and Communication need or Eating Drinking and Swallowing need.
Please note that further advice or recommendations may be given before a referral is accepted.

Preschool Speech and Language Therapy Referrals
Professionals: To make a referral, simply choose the child’s age group below and download the appropriate form to complete.
- 18 months - 2 years old referral form.
- 2 years - 3 years old referral form.
- 3 years - 4 years old referral form.
Parents: Please get in touch with our admin team to complete a telephone referral form on 01384 324675.
Please note: If you are re-referring a child who has been seen by Dudley Speech and Language Therapy service in the last six months, you do not need to complete these forms. Please email/phone the department to discuss the re-referral.
School Age Speech and Language Therapy Referrals
Professionals: Please choose the approproiate form below and return to the address/email listed above. Please ensure you have completed all sections of the referral request form. Pages 4-6 must be completed in conjunction with the child/young person's educational setting, and evidence for the assess/plan/do/review model must be included. If the required evidence is not submitted with the referral request form, we will return the referral form to you.
